Principal Investigator, TRISVR: Baron Hamman, M.D. Location: Baylor University Medical Center Sponsor: Chase Medical (TRISVR), NIH (STICH) Primary Objective: The purpose of this study is to capture a comprehensive set of multi-center prospective data, including baseline quality of life, pre-operative, operative, and post-operative clinical outcome data, and follow-up quality of life and clinical data for patients who undergo the TR3ISVR procedure. These heart failure patients undergo CABG + or - Left Ventricular Remodeling. They are enrolled in STICH at the same time.
