Telephone Referrals: Each office is staffed by scheduling personnel who will be happy to make appointment requests via telephone. Just call the appropriate office with the patient's name, contact information, and the desired service to be performed (consultation, test). Fax Referrals: Some of the HeartPlace offices offer a fax referral option. If you see the link below to the particular office you wish to refer, you may click on that link and download the Referral form specific for that office. Online Referrals:  Some of the HeartPlace offices offer a internet-based referral option by way of our secure Referral Request portal. Each referring physician is assigned a unique username and password to access the system. This is a secure system designed to allow the transfer of protected private personal information. Call the specific HeartPlace office you use to see if they are participating yet in the Referring Physician Portal option. If they are, you may request a username and password and begin referring patients on-line today.
